  Psoriasis, commonly known as "psoriasis", is a long-lasting, stubborn, and recurrent skin disease. Clinically, it is characterized by scattered red papules or plaques on the skin, covered with silver-white scales, which is a chronic recurrent erythema squamous skin disease. There are mainly several types: common type, erythrodermic type, pustular type, and articular type. The common type of psoriasis is divided into localized type and disseminated type (generalized type). At present, in the field of traditional Chinese medicine, the treatment of generalized common psoriasis in the stationary phase is based on blood deficiency and wind dryness, mainly nourishing blood and moistening the skin, supplemented by clearing heat and cooling blood; in the progressive phase, it is based on blood heat and wind dryness, mainly clearing heat and cooling blood, supplemented by nourishing blood and moistening dryness. In both phases, if there is itching, add wind-dispelling and itching-relieving herbs, and for long-term diseases, use insect-type wind-searching drugs. This is the common method and method for treating psoriasis, which has good curative effect, but the cure rate is not high, only 30%~60%. The root cause is still not deep enough understanding of the pathogenesis, etiology, and pathogenesis of psoriasis. It is far from enough to summarize the pathogenesis of psoriasis as blood dryness as the root and wind toxin as the branch. This article discusses the treatment of psoriasis from the perspective of the five elements and the four seasons, and aims to improve the curative effect and benefit the people.

  The five elements and the four seasons are as follows:

  Spring: wood (liver) is prosperous, fire (heart) is auxiliary, water (kidney) is dormant, metal (lung) is囚, and earth (spleen) is dead.

  Summer: wood is dormant, fire is prosperous, water is囚, metal is dead, and earth is auxiliary.

  Autumn: wood is dead, fire is囚, water is auxiliary, metal is prosperous, and earth is dormant.

  Winter: wood is auxiliary, fire is dead, water is prosperous, metal is dormant, and earth is囚.

  Spring is the months of Yin and Mao, summer is the months of Si and Wu, autumn is the months of Shen and You, and winter is the months of Hai and Zi. When the season is in power, it is prosperous; when the season is born, it is auxiliary; when the season is born, it is dormant; when the season is restrained, it is囚; when the season is restrained, it is dead.

  Prosperous: the most prosperous, the one in power governs in person, holding great power, just like an emperor.

  Auxiliary: the second most prosperous, favored by the emperor, just like a prime minister.

  Dormant: the power is slightly declining, heading towards decline, resting in peace.

  Imprisoned: the power is moderately declining, imprisoned.

  Dead: the most declining, heading towards destruction.

  The five elements should be balanced and harmonized. The five elements correspond to the five internal organs. The imbalance of the original qi of the internal organs can lead to diseases.

  Chronic generalized common psoriasis can occur in all seasons, but mainly has three manifestations: (1) aggravated at the end of winter and early spring, and mitigated in summer; (2) aggravated in winter and mitigated in summer; (3) aggravated in spring and summer, and mitigated in winter.

  Psoriasis can occur in all seasons. The treatment of psoriasis should follow the theory of "Zangqi Fashi Lun" in the "Neijing", that is, the qi of the five internal organs follows the four seasons. There are five elements within the five elements, and four seasons within the four seasons. Adhere to the pathogenesis of the disease, and the stubborn psoriasis will be effective.

  First, aggravated at the end of winter and early spring, and mitigated in summer.

  Patients with this type of psoriasis often relapse at the end of winter and early spring, with rapid spread of rash, red rash, and itching. In summer, the symptoms may alleviate or subside, and in autumn and winter, they are in the stationary phase, with stable condition, no further expansion of skin lesions, dry skin, pale rash, thin scales, slight itching or almost no itching. This is because in autumn, metal is prosperous and water is auxiliary, and in winter, water is prosperous and wood is auxiliary. Metal can收敛 and water can涵木, so wind and wood do not move, and fire does not flame, resulting in stable condition. However, at the end of winter and early spring, the wood qi gradually becomes prosperous. In spring, wood is prosperous and fire is auxiliary, wood and fire promote each other, the human body's wood and fire tend to be dispersive and ventilative, metal and water are dormant and囚, metal does not降fire, water does not涵wood, so wind heat accumulates in the surface and old diseases relapse. In winter and spring, the spleen earth is imprisoned in the dead place, lung metal is off duty and囚, earth deficiency leads to wood invasion, wind wood dispersive too much, and earth does not generate metal, metal does not restrain wood, lung qi guards the exterior not solid, so the rash spreads rapidly. In the season, earth is prosperous and wood is囚, fire is dormant, wind heat evil decreases, and in summer, fire is prosperous and earth is auxiliary, wood is dormant, so the condition alleviates or even subsides. The earth in spring is the most isolated, afraid of wood cutting. The basic pathogenesis of psoriasis aggravated at the end of winter and early spring, and mitigated in summer is "earth deficiency and wood invasion", and long-term diseases will cause blood stasis, accumulation of blood stasis into poison. Spleen and lung qi deficiency, then the force to promote blood flow is insufficient, resulting in poor blood flow and stasis. The treatment should suppress wood and support earth, taking both the root and branch, that is, invigorating spleen and replenishing qi, dispelling wind and removing dampness, clearing heat and cooling blood, promoting blood circulation and removing blood stasis.

  (I) Internal treatment (experienced prescription)

  "Wushe Yuyin Decoction": Wushe 30 grams (swallowed), Huangqi 30-60 grams, Dangshen 15-20 grams, fried Baizhu 12 grams, Fangfeng 10 grams, Cijili 15 grams, Zhi Shouwu 15 grams, Baixianpi 10 grams, Quanshen 10 grams, Taoren 10 grams, Honghua 10 grams, Danggui 10 grams, Chuanxiong 10 grams, Jixueteng 15 grams, decocted in water.

  This prescription is derived from the combination of "Wushe Rongpi Decoction" and " Yupingfeng Powder", nourishing blood and dispelling wind to relieve itching, replenishing qi and promoting blood circulation to remove blood stasis. For long-term and recurrent diseases, add Jingjie 10 grams and Zhaohuacao 10 grams, entering the blood, expelling evil from the outside, and adding Langdu 3 grams, Quanxie 3 grams, and Wugong 2, attacking poison and dispersing stagnation. For spleen deficiency and loose stools, remove Shouwu, Taoren, and Danggui, and add Cangzhu 10 grams. If spleen deficiency is accompanied by cold, diarrhea or loose stools after catching cold, remove Baixianpi, Quanshen, and Danshen and other cold and cool herbs, and add Ganjiang 6-10 grams and Rougui 6-10 grams to warm and transport spleen yang. Long-term diseases will cause blood stasis. If the tongue is purple and dark, the lips are purple and dark, there are blood stasis spots on the tongue coating, the patches are thick and hard, and the patch color is dark red, this is blood stasis, which should break blood and remove blood stasis, add Danshen 15 grams, Zhifulang 6 grams, Zhimoyao 6 grams, Sanleng 6 grams, and Ezhu 6 grams.

  (II) External treatment (experienced prescription)

  1.   Quyin Ointment: mainly composed of Yangti, Tujinpi, Baixianpi, Kushen, Zhaohuacao, Lufengfang, Cihuirou, Majiangzi, Shetui, Quanxie, Langdu, Shuizhi, Wugong, Xionghuang, and other 21 herbs, fried in sesame oil, and collected with yellow wax. Take the ointment and rub the affected area once a day. In late spring and early summer, if the rash is red, remove Zhaohuacao and add Dahuang and Huangbai. This ointment can also be used for the type with aggravation in spring and summer and mitigation in winter.

  2.   Baidu Ointment: made of realgar, xionghuang, talcum powder, red Shengdan, and Xingshi (arsenic trioxide), all ground into powder. Quanxie and Wugong are fried in sesame oil until withered, filtered to remove the residue, and then mixed with the remaining medicine powder, and collected with yellow wax. Take the ointment and rub the affected area once a day. This ointment is used alternately with "Quyin Ointment", complementing each other, playing a "wheel battle", to attack poison and disperse stagnation. Allergic constitution and those who are allergic to mercury, arsenic, and sulfur are prohibited from using it.

  3.   Xiaoyin Lotion: Yangtigen 30 grams, Kushen 30 grams, Baixianpi 20 grams, Heluo 15 grams, Shechuangzi 15 grams, Difuizi 20 grams, Tougucao 30 grams, Zhaohuacao 15 grams, Chantui 15 grams, Fengfang 10 grams, Dafengzi 15 grams, Mubiezi 15 grams, Langdu 10 grams, Xingren 10 grams, Tulinfu 30 grams, Quanshen 15 grams, Cijili 15 grams, Jingjie 15 grams, decocted for bathing, once a day or every other day. If the itching is severe, add Bingpian 3 grams and Bohe Nao 3 grams, and add them to the medicine water for bathing.

  4.   Huaban Ointment: mainly composed of Wubeizi, Wangbuliuxing, and Tuanyuan, cooked into paste with vinegar and honey. Take the ointment and rub the affected area once a day. This ointment is used for psoriasis with infiltration, thickening, and hard patches, and has the effect of softening the patches.

  Case 1: Zhang Mou, male, 11 years old, from Gucheng, Feidong, Anhui. He suffered from psoriasis since he was three or four years old and had been suffering for eight or nine years. He sought treatment everywhere, and although he had been treated by experts from major hospitals in Hefei, Nanjing, Shanghai, and other places, both Chinese and Western medicine had been tried, and he had spent a lot of money on medical expenses, but the effect was not satisfactory. Every summer, the symptoms would alleviate or subside, and in autumn and winter, the attacks would not spread. However, at the end of winter and the beginning of spring, the rash would begin to spread, especially in spring, with scattered papules, erythema, and scales all over the body, limbs, chest, abdomen, head, and face, with this subsiding and that rising, and the rash was itchy. In late January 2011, the patient was diagnosed with a cluster of papules and patches on the left lower leg tibia, with pale color, dryness, like pine bark, hard, covered with scales, and scattered papules and patches on the buttocks and back, slightly itchy. According to the common method of treatment in the stationary phase, the syndrome was diagnosed as blood deficiency and wind dryness, and the treatment was mainly to dispel wind and nourish blood. After one month of treatment, the effect was not significant. By the end of February, there was a sign of spreading, with scattered new skin lesions appearing, red papules and patches, and itching. After the Spring Festival, the rash began to spread, with scattered papules and scales visible on the buttocks, chest, waist, and head. The long-term evil poison accumulated, penetrated into the blood, and it was difficult to achieve a good effect without removing the poison from the outside. According to the common method of treatment in the progressive phase, the syndrome was diagnosed as blood heat and wind dryness, and the treatment was mainly to clear heat and cool blood, supplemented by nourishing blood and moistening dryness. Externally, take a bath with "Qingre Zhiyang Decoction", and take "Wushe Rongpi Decoction" internally with Jingjie, Zhaohuacao, Niubangzi, Chantui, Yinhuagen, and Lianqiao. After taking 3 doses of the medicine, new skin lesions continued to increase, which was a sign of expelling evil from the outside. However, the patient could not tolerate the cold and cool nature of the medicine, and suffered from diarrhea after taking the medicine, so the internal medicine was changed to external treatment. The parents of the patient were worried and were told not to worry. Continue to treat with the original prescription, after half a month of treatment, new skin lesions no longer appeared, some of the existing skin lesions subsided, and the condition was controlled, but not fundamentally improved.
